چکیده

Background Abnormal aortic arch geometry, in particular the ‘gothic arch’, has been associated with reduced exercise performance in patients with repaired aortic coarctation. Following the arterial switch operation (ASO) for transposition of the great arteries (TGA), the morphology of the aorta is also known to be abnormal, presenting with dilated aortic root and ‘gothic’ deformity of the aortic arch. In this study, our aim was to compare aortic geometry between normal and ASO patients to establish if aortic arch shape influenced exercise performance.

Aortic arch shape is not associated with hypertensive response to exercise in patients with repaired congenital heart diseases

BACKGROUND Aortic arch geometry is linked to abnormal blood pressure (BP) response to maximum exercise. This study aims to quantitatively assess whether aortic arch geometry plays a role in blood pressure (BP) response to exercise. METHODS 60 age- and BSA-matched subjects--20 post-aortic coarctation (CoA) repair, 20 transposition of great arteries post arterial switch operation (ASO) and 20 h...

Mechanical and morphological properties of the aortic root and arch late after arterial switch operation for transposition of the great arteries

Background The arterial Switch operation (ASO) is now performed routinely for the repair of transposition of the great arteries (TGA). Neo-aortic root dilatation and reduced elasticity of the ascending aorta have been shown to impact the long-term outcomes of these patients. The aim of this study was to assess the effect of the ASO with the Lecompte maneuver on the dynamic behavior and dimensio...

Implications of early aortic stiffening in patients with transposition of the great arteries after arterial switch operation.

BACKGROUND The elastic function of the aorta in patients with transposition of the great arteries after arterial switch operation (ASO) is suspected to be important for long-term prognosis. METHODS AND RESULTS Fifty-one patients and 34 controls were studied at 3.0 Tesla with MRI. Forty-three patients (12.8±6.9 years) underwent 1-stage ASO, 8 patients (23.8±6.9 years) had prior pulmonary arter...

Surgical strategy in transposition of the great arteries with aortic arch obstruction.

OBJECTIVE To analyze our experience in the surgical correction of transposition of the great arteries associated with aortic arch obstruction. METHOD From January 1998 to December 2005 we performed 223 arterial switch operations for transposition of the great arteries; 21 (9.4%) patients had associated aortic arch obstruction. Aortic arch anatomy showed: localized aortic coarctation (n=10) an...

Exercise performance and activity level in children with transposition of the great arteries treated by the arterial switch operation.

The exercise capacity of children after arterial switch for transposition of the great arteries (TGA) is known to be at the lower limit of normal. We aimed to ascertain whether this results from compromised hemodynamics or deconditioning. A total of 17 children with TGA (12 male and 5 female children; age 12.1 + or - 2.0 years) treated with the arterial switch operation were compared with 20 ag...
